10/1/2023 EnchantaVaughn Approved QMHPA greeted Ms. Brown upon arrival to the home setting. Upon arrival to the home setting, Ms. Brown appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her afternoon. Ms. Brown verbalized to QMHPA that her afternoon has been pleasant. QMHPA informed Ms. Brown the focus on this session was going to focus on being able to manage her frustrations and make positive decision making skills associated with those frustrations. Ms. Brown did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Ms. Brownâs current thoughts, feelings and emotions. QMHPA educated Ms. Brown how to identify the purpose of her decision(s), identify Standards and Judgments while gathering information to determine alternatives and actions and then evaluate the outcome of that decision. QMHPA engaged Ms. Brown in a skills building activity. QMHPA explained to Ms. Brown that the purpose of this activity was to assist Ms. Brown with applying critical thinking as a skill she can use to make good choices and decisions. QMHPA engaged Ms. Brown in another skills building activity. QMHPA explained to Ms. Brown that the purpose of this skills was to teach Ms. Brown how to express her thoughts and feelings in order to meet her needs. QMHPA also informed Ms. Brown that this skill will assist her with building confidence with meeting her needs in a timely manner. QMHPA assisted Ms. Brown with identifying how her anger impacts others and identifying ways to channel that energy in a positive direction. Ms. Brown appeared to be receptive to QMHPAâs interventions and services that were provided to her during the session. Ms. Brown acknowledged that she has a problem with making decisions especially if they are crucial to her life. Ms. Brown stated, âI let my emotions get the best of me and I think I have to challenge everything. I get in trouble for trying to tell people what to doâ. Ms. Brown was encouraged to continue using critical thinking to help her make good decisions in stressful situations. Ms. Brown shared that she felt her judgment was good/bad. She verbalized to QMHPA that she feels as though it is bad at times because she allows others to make her mad and take her to another place that she really doesnât need to be. She further discussed with QMHPA that when she is taking to this place, she is not happy and she is very depressed. Ms. Brown was encouraged to continue finding ways to make good decisions. the time she said she will be good at one moment talking and joking around then she will just become angry.
